For the vaccination of 12-17 yrs old WITHOUT COMORBIDITIES:
The vaccinee MUST BE ACCOMPANIED BY an adult (parent/guardian)Bring the requirements for vaccination:a. any valid ID card of the vaccinee and the accompanying parent/guardian
b. documentary proof of FILIATION (any document bearing the name of the adult in relation to the child)
Examples of DOCUMENTS TO PROVE FILIATION:
-Birth cert of minor issued by PSA or LCR
-baptismal cert with the name of parent/guardian
-school ID or any record/documents of the child bearing the name of the parent/guardian
-any document indicating that the vaccine recipient is a child of the parent
-letter of authorization if parents are abroad
